Read before Signing – Whereas the minor will hereafter be referred to as ‘the participant’. Acknowledgement of Risks and Responsibilities for Drop-in Skate Card (Figure Skating) Programming: 1. The activities involved in the program are dangerous. The participant risks the chance of serious injury or death through participation. 2. I believe that the participant is physically, emotionally and mentally able to fully participate in the program, and as such have given my unqualified permission for the participant to take part. 3. I will ensure the participant is familiar with and will follow all the applicable rules for participation in the program. 4. The participants equipment is mechanically fit and suitable for use in the program. 5. I understand that at all times during participation in the program, I have sole responsibility for the participants’ safety. 6. If, during the course of participation in the program: A. I learn of or become aware of a change in the participants’ health, physical, emotional, or mental condition, or B. the participant feels unsafe or threatened for any reason, or C. the participant notices anything unsafe about the program, the participant will immediately stop participating and I take full responsibility for the participants actions and behaviour. 8. I have read this statement of risks and responsibilities. I understand and agree with what I have read and choose to sign.
